Pathfinder
About this service
Pathfinder Stockport is provided by Pennine Care NHS Trust. The service helps people identify their needs to successfully recover from drug and alcohol misuse. This service works with those who are more likely to need long term support and treatment. This can include prescribing and community detox as well as care planned support and group work. Pathfinder work closely with individuals and their families offering them the right support to help with a successful recovery of those close to them.
They do this by offering:
- Group work, providing peer support and peer challenges
- Partnerships with the Lifeline project
- Intuitive thinking skills
- One-to-one support
- Links to mutual aid groups and wider organisations
The programmes are aimed at supporting people to set goals and targets to achieve to be able to recover successfully.
